Registration Process:

1. Complete the Registration Form below
2. Pay the Registration Fee ($200)

3. Complete and return the Volunteer and Medical Forms. Per ASP national requirements, the Volunteer form must be notarized. Most bank branches have notaries.
4. Complete and return the CORI form along with a copy of your driver’s license.
5. Opt into our Mailchimp Mailing List and add info@cohassetasp.org to your address book. We send regular and important updates to this list.
